Our Electronic Health Records company, Revolution EHR, offers a virtual chart for patients to view their exam notes and finalized, non-expired contact lens and glasses prescriptions.

If you did not receive instructions on how to log into your virtual chart please call the office to make sure your virtual chart has been enabled by our staff.

The login should be your email address and the temporary password is set to your 8 digit date of birth with no dashes, spaces, or special characters.
